THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

When this Notice of Privacy Practices (“Notice”) refers to “we” or “us,” it is referring to Elevate Wellness Center and all the pharmacists who provide health care services and the employees of our pharmacy. We are required by law to maintain the privacy of your protected health information (“PHI”), to follow the terms of the Notice currently in effect, to give you this Notice setting forth our legal duties and privacy practices concerning your PHI, and to notify affected individuals following a breach of unsecured PHI.

We reserve the right to amend this Notice. If we make material revisions, we will post a copy of the revised Notice in the pharmacy and on our website.


I. USE AND DISCLOSURE OF YOUR PHI

We will use and disclose your PHI for treatment, payment, and health care operations. Other uses not described here will only be made with your explicit written authorization, which you may revoke at any time.

A. Treatment – We may use and disclose your PHI to provide you with prescription and supply services. This includes disclosure to other pharmacists, technicians, and health care providers involved in your care. B. Payment – We use your PHI to obtain payment for services provided, including disclosures to your health plan for prior approval or coverage determination. C. Health Care Operations – We may use your PHI for pharmacy management, such as quality assessment, internal audits, and performance evaluations. D. Reminders and Benefits – We may contact you for prescription refill reminders, treatment alternatives, or health-related benefits that may interest you. E. Family and Friends – Unless you object, we may disclose PHI to family members or friends involved in your medical care or payment for care. F. Other Permitted/Required Uses – We may use PHI without your authorization as required by law for:

  • Public health activities (preventing disease, reporting adverse reactions).

  • Health oversight activities (audits, investigations).

  • Judicial/administrative proceedings (subpoenas, court orders).

  • Law enforcement purposes.

  • Coroners, medical examiners, and funeral directors.

  • Organ procurement and research.

  • Averting a serious threat to health or safety.

  • Military, national security, and protective services.

  • Workers' compensation.


II. YOUR RIGHTS AS OUR PATIENT

A. Request Restrictions – You have the right to request limitations on how we use or disclose your PHI. We are not required to agree unless the request involves a transaction you paid for in full out-of-pocket. B. Confidential Communications – You may request to receive communications by alternative means or at alternative locations (e.g., a different mailing address). C. Access and Copies – You have the right to inspect and obtain a copy of your PHI (including electronic versions). We may charge a reasonable, cost-based fee for copies and postage. D. Accounting of Disclosures – You may request a list of certain disclosures we have made of your PHI for the past six years. One request per year is free. E. Right to Amend – If you believe your PHI is incorrect or incomplete, you may request an amendment. We may deny this if we determine the information is already accurate or was not created by us. F. Paper Copy – You have the right to a paper copy of this Notice at any time. G. Fundraising – You have the right to opt-out of fundraising, and your PHI will not be sold without your authorization.

What is Elevate Wellness Center and how does it work?

Elevate Wellness Center provides wellness education, coaching, and care coordination to support individuals in navigating their health goals. We focus on guidance, accountability, and helping you connect with appropriate clinical care when needed.

Medical evaluations and prescribing decisions are performed by independent licensed healthcare providers.

How does the process work?

You begin by completing an intake and sharing your goals. If appropriate, an independent licensed healthcare provider may review your information and determine whether treatment is clinically suitable. Elevate then provides ongoing coaching and coordination support alongside your care plan.

Do I need to book a consultation?

Consultations are not required to begin the process. However, if you would like to schedule a session to review your goals, ask questions, or receive guidance and support, coaching consultations are always available and can be booked at your convenience. Coaching sessions may be scheduled to discuss goals, education, and support. If clinical evaluation is appropriate, you may be evaluated by an independent licensed healthcare provider as part of the process.

Are prescriptions guaranteed?

No. Treatment and prescribing decisions are made solely by licensed healthcare providers based on clinical judgment, medical history, and appropriateness. Not all individuals will qualify for treatment.

How are medications shipped?

If prescribed, medications are dispensed and shipped directly by independent licensed pharmacies. Shipping timelines and processing are determined by the pharmacy and may vary. In many cases, medications arrive within approximately 2–5 business days after pharmacy processing.

Can I add optional boosters?

In some cases, providers may offer additional therapies, peptides, or supportive options when clinically appropriate. These decisions are made by the licensed healthcare provider based on your individual needs, goals, and medical history.

Do you take insurance?

Elevate Wellness Center services are typically self-pay. Coverage for provider services, medications, or pharmacy fulfillment varies and should be discussed directly with your provider or pharmacy.

Payment plan options may be available for certain services or programs.

What if I have side effects or questions?

Medical questions, medication concerns, or potential side effects should be directed to your prescribing healthcare provider or dispensing pharmacy. You will be provided with contact information, including a phone number to text your medical provider regarding side effects, medical questions, or concerns when appropriate.

Where are you licensed to operate?

All medications provided through the program are compounded pursuant to patient-specific prescriptions prepared by licensed pharmacies. Compounded medications are customized for individual patients and are not reviewed or approved by the U.S. Food and Drug Administration (FDA) in the same manner as commercially manufactured medications.

Compounding pharmacies are regulated primarily by state boards of pharmacy and must follow applicable quality and safety standards. You should discuss risks and benefits directly with your provider and pharmacy.

What is compounded medication and how safe is it?

Compounded medications are custom-prepared by licensed pharmacies based on an individual prescription. They may not carry FDA approval but are produced under stringent regulatory standards in FDA-inspected facilities. They are only prescribed when deemed appropriate by your provider.

Is there a refund policy?

Due to the individualized nature of services and prescription products, payments are generally non-refundable once services are rendered or orders are processed. Please refer to the Refund & Cancellation Policy for full details.

What should I do in an emergency?

Shipping timelines vary depending on pharmacy processing and delivery logistics. Many prescriptions are delivered within approximately 2–5 business days, although timing may vary based on pharmacy workload, medication preparation, and shipping conditions.

Who prescribes the medications?

If you experience severe symptoms, a concerning reaction, or a medical emergency, call 911 or seek immediate emergency medical care. Coaching services and electronic communication are not appropriate for urgent or emergency situations.

Are these medications FDA-approved?

Medications are prescribed solely by independent licensed healthcare providers who determine treatment appropriateness based on clinical evaluation.

Elevate Wellness Center does not prescribe medications.

Is my information secure?

Elevate Wellness Center maintains safeguards designed to protect your information and uses secure systems for communication and coordination. Independent providers and pharmacies maintain their own privacy protections as well.
