Medical evaluations, diagnoses, treatment decisions, laboratory ordering, and prescribing are performed exclusively by independent licensed healthcare providers operating through third-party medical groups. These providers exercise independent professional judgment and are solely responsible for determining whether services, treatments, or prescriptions are clinically appropriate based on the individual patient’s medical history, presentation, and applicable regulations.

No provider–patient relationship is created between you and Elevate Wellness Center LLC. Any provider–patient relationship that may be established exists solely between you and the independent licensed healthcare provider delivering clinical services.
Providers may only deliver services within jurisdictions where they are properly licensed and authorized to practice. Eligibility for medical services may vary based on provider licensure, clinical appropriateness, medical history, and regulatory requirements. Not all individuals are appropriate candidates for all therapies, and providers may decline care or treatment when clinically indicated.
Medical evaluations and prescribing decisions are performed exclusively by independent licensed healthcare providers based on clinical judgment, applicable laws, and professional standards of care. There is no guarantee that any individual will receive treatment, medication recommendations, or a prescription.
If a provider determines that treatment is appropriate, prescriptions may be transmitted to licensed pharmacies selected by the provider or the patient where permitted by law. Patients may have the ability to request prescription transmission to a pharmacy of their choosing in accordance with applicable regulations and provider discretion.

MEDICATION FULFILLMENT & COMPOUNDING DISCLOSURE
If an independent licensed healthcare provider determines that treatment is appropriate, prescriptions may be transmitted to a pharmacy selected by the provider or the patient where permitted by law.
Pharmacies involved in medication fulfillment operate independently and are responsible for medication preparation, dispensing, shipping, and patient counseling. Elevate Wellness Center LLC does not operate pharmacies, dispense medications, compound pharmaceutical products, or control pharmacy fulfillment processes.
Some prescriptions may be fulfilled by state-licensed compounding pharmacies preparing medications pursuant to patient-specific prescriptions. Compounded medications, including compounded GLP-1 therapies, are not reviewed or approved by the U.S. Food and Drug Administration (FDA) in the same manner as commercially manufactured medications.
Patients are encouraged to discuss medication risks, benefits, alternatives, and pharmacy-related questions directly with their prescribing provider and dispensing pharmacy.
Elevate Wellness Center provides wellness education, coaching, and care coordination to support individuals in navigating their health goals. We focus on guidance, accountability, and helping you connect with appropriate clinical care when needed.
Medical evaluations and prescribing decisions are performed by independent licensed healthcare providers.
You begin by completing an intake and sharing your goals. If appropriate, an independent licensed healthcare provider may review your information and determine whether treatment is clinically suitable. Elevate then provides ongoing coaching and coordination support alongside your care plan.
Consultations are not required to begin the process. However, if you would like to schedule a session to review your goals, ask questions, or receive guidance and support, coaching consultations are always available and can be booked at your convenience. Coaching sessions may be scheduled to discuss goals, education, and support. If clinical evaluation is appropriate, you may be evaluated by an independent licensed healthcare provider as part of the process.
No. Treatment and prescribing decisions are made solely by licensed healthcare providers based on clinical judgment, medical history, and appropriateness. Not all individuals will qualify for treatment.
If prescribed, medications are dispensed and shipped directly by independent licensed pharmacies. Shipping timelines and processing are determined by the pharmacy and may vary. In many cases, medications arrive within approximately 2–5 business days after pharmacy processing.
In some cases, providers may offer additional therapies, peptides, or supportive options when clinically appropriate. These decisions are made by the licensed healthcare provider based on your individual needs, goals, and medical history.
Elevate Wellness Center services are typically self-pay. Coverage for provider services, medications, or pharmacy fulfillment varies and should be discussed directly with your provider or pharmacy.
Payment plan options may be available for certain services or programs.
Medical questions, medication concerns, or potential side effects should be directed to your prescribing healthcare provider or dispensing pharmacy. You will be provided with contact information, including a phone number to text your medical provider regarding side effects, medical questions, or concerns when appropriate.
All medications provided through the program are compounded pursuant to patient-specific prescriptions prepared by licensed pharmacies. Compounded medications are customized for individual patients and are not reviewed or approved by the U.S. Food and Drug Administration (FDA) in the same manner as commercially manufactured medications.
Compounding pharmacies are regulated primarily by state boards of pharmacy and must follow applicable quality and safety standards. You should discuss risks and benefits directly with your provider and pharmacy.
Compounded medications are custom-prepared by licensed pharmacies based on an individual prescription. They may not carry FDA approval but are produced under stringent regulatory standards in FDA-inspected facilities. They are only prescribed when deemed appropriate by your provider.
Due to the individualized nature of services and prescription products, payments are generally non-refundable once services are rendered or orders are processed. Please refer to the Refund & Cancellation Policy for full details.
Shipping timelines vary depending on pharmacy processing and delivery logistics. Many prescriptions are delivered within approximately 2–5 business days, although timing may vary based on pharmacy workload, medication preparation, and shipping conditions.
If you experience severe symptoms, a concerning reaction, or a medical emergency, call 911 or seek immediate emergency medical care. Coaching services and electronic communication are not appropriate for urgent or emergency situations.
