Elevate Wellness Center, LLC is not a medical practice and does not provide clinical care or make medical decisions. We operate as a Management Services Organization (MSO), offering administrative, technological, and coordination support. All clinical services are delivered solely by independently contracted, licensed medical providers.

2. Nature of Telehealth Services

  • Prescription of medications, including compounded drugs

Services are delivered through HIPAA-compliant platforms including secure video conferencing, telephone, and/or asynchronous communications (e.g., messaging, emails, photos).

Providers will rely on the information you submit—such as intake forms, health history, photos, and other documentation—to make treatment decisions.

3. Role of Elevate Wellness Center
Elevate Wellness Center, LLC:

  • Is not a medical provider or prescriber

  • Does not engage in the practice of medicine

  • Does not guarantee prescriptions or medical treatments

All healthcare services are provided by licensed, independent medical professionals, contracted through our affiliated provider networks. These providers have full clinical autonomy and may decline to prescribe medication or offer treatment if not deemed medically appropriate.

5. Limitations & Risks of Telehealth
Technology Risks: Service interruptions due to poor internet connectivity; potential unauthorized access despite HIPAA-compliant safeguards.

Clinical Limitations: Absence of a physical exam may limit a provider’s ability to fully evaluate your condition; certain health issues may require in-person evaluation or lab testing.

Prescription Limitations: No guarantee that medications (including GLP-1s or compounded products) will be prescribed; prescriptions are based on provider judgment and clinical necessity.

6. Medications & Compounded Drugs
If prescribed, your provider may recommend:

  • FDA-approved medications, or

  • Compounded medications prepared by licensed, FDA-registered compounding pharmacies

Important Notes:

  • Compounded medications are not FDA-approved for safety or efficacy

  • They are formulated for individual patients when commercial drugs are unavailable or unsuitable

  • Providers may decline to prescribe compounded medications based on clinical appropriateness

7. Privacy & Confidentiality
Your information is protected under HIPAA and applicable state/federal privacy laws. Telehealth sessions may be recorded for internal quality assurance and training. Elevate Wellness Center uses secure, HIPAA-compliant technologies. You are responsible for ensuring your own privacy during sessions (e.g., choosing a quiet, secure space).

8. Communication & Technology Requirements
A stable internet connection and access to a video-enabled device are required. You consent to receive communications (including appointment reminders) via email, phone, or text. Elevate Wellness Center is not responsible for technical issues, lost messages, or data costs.

9. Financial Responsibility
All services are self-pay; insurance is not accepted. You are responsible for all out-of-pocket costs, including consultations, medications, and follow-ups. Payment is due at the time of service. Additional policies regarding late cancellations or no-shows are available upon request.

10. Emergencies & After-Hours Care
Telehealth is not appropriate for emergencies. If you are experiencing a life-threatening emergency, call 911 or go to your nearest emergency room.

If you are experiencing severe side effects from a prescribed medication, contact your provider or seek immediate in-person medical care.

11. Patient Rights & Responsibilities
You have the right to:

  • Ask questions about your treatment and medications

  • Decline or withdraw from telehealth services at any time

  • Expect confidentiality, professionalism, and respect from all providers

You agree to:

  • Provide complete and accurate health information

  • Notify your provider of any changes to your condition

  • Follow treatment recommendations and not misuse prescribed medications

  • Participate in sessions from a secure, private environment

However, if you would like to schedule a session to review your goals, ask questions, or receive guidance and support, coaching consultations are always available and can be booked at your convenience. Coaching sessions may be scheduled to discuss goals, education, and support. If clinical evaluation is appropriate, you may be evaluated by an independent licensed healthcare provider as part of the process.

Are prescriptions guaranteed?

No. Treatment and prescribing decisions are made solely by licensed healthcare providers based on clinical judgment, medical history, and appropriateness. Not all individuals will qualify for treatment.

How are medications shipped?

If prescribed, medications are dispensed and shipped directly by independent licensed pharmacies. Shipping timelines and processing are determined by the pharmacy and may vary. In many cases, medications arrive within approximately 2–5 business days after pharmacy processing.

Can I add optional boosters?

In some cases, providers may offer additional therapies, peptides, or supportive options when clinically appropriate. These decisions are made by the licensed healthcare provider based on your individual needs, goals, and medical history.

Do you take insurance?

Elevate Wellness Center services are typically self-pay. Coverage for provider services, medications, or pharmacy fulfillment varies and should be discussed directly with your provider or pharmacy.

Payment plan options may be available for certain services or programs.

What if I have side effects or questions?

Medical questions, medication concerns, or potential side effects should be directed to your prescribing healthcare provider or dispensing pharmacy. You will be provided with contact information, including a phone number to text your medical provider regarding side effects, medical questions, or concerns when appropriate.

Where are you licensed to operate?

All medications provided through the program are compounded pursuant to patient-specific prescriptions prepared by licensed pharmacies. Compounded medications are customized for individual patients and are not reviewed or approved by the U.S. Food and Drug Administration (FDA) in the same manner as commercially manufactured medications.

Who prescribes the medications?

If you experience severe symptoms, a concerning reaction, or a medical emergency, call 911 or seek immediate emergency medical care. Coaching services and electronic communication are not appropriate for urgent or emergency situations.

Are these medications FDA-approved?

Medications are prescribed solely by independent licensed healthcare providers who determine treatment appropriateness based on clinical evaluation.

Elevate Wellness Center does not prescribe medications.

Is my information secure?
